Mortgage Loan Application Checklist

Information About You:

  • Copy of your Driver’s License or other government issued photo ID
  • 2 years of W-2s
  • Paystubs for the last 30 days
  • Last two months of bank statements for checking and savings – all pages
  • Most recent quarterly copies of 401(k), pension, and/ or other investment account statements – all pages

Information About the Home:

  • Copy of accepted Offer to Purchase, include amendments, counter offers, and Property Condition Report
  • Homeowner’s Insurance Declaration Page
  • Homeowner’s Insurance once year paid receipt, or invoice if paying at closing

For new construction

  • Construction contract, blueprints, and bid specification
  • Purchasing the Lot: copy of Offer to Purchase
  • Own the Lot: copy of Closing Disclosure for date of purchase

Special Circumstances:

  • Selling your present home: copy of the listing contract, accepted Offer to Purchase, and Closing Disclosure at time of sale
  • Self-employed or Commission-based pay: 2 years of personal and business tax returns with K-1s
  • Newly employed or job relocation: copy of signed Offer of Employment
  • Divorced: copy of divorce decree and proof of receipt of child support, alimony and/ or spousal support
  • Own investment property: copies of current rental leases
  • Proof of military status/ service DO214. Copy #4
  • Receiving an award: copy of award letter (social security, unemployment, social security disability)
